We are seeking a detail-oriented and motivated Data Quality Care Coordinator to join our friendly Care Coordination Team at Carlton Group Practice. This is a vital role in ensuring the accuracy and quality of patient records, supporting clinical care, audits, and national reporting requirements such as QOF.

Job Purpose:

To ensure the accurate and timely coding of clinical information into patient records using Read Codes and/or SNOMED CT, supporting high standards of data quality, clinical audits, and compliance with national frameworks such as QOF. The role also contributes to the continuous improvement of digital healthcare delivery by supporting the integration and use of innovative tools, including AI-driven platforms such as Anima, to enhance patient care, streamline workflows, and improve clinical efficiency.

  • Accurately code clinical data from consultations, hospital letters, and other documentation.
  • Summarise patient records and ensure relevant information is coded appropriately.
  • Support the practice in achieving QOF and Enhanced Services targets.
  • Maintain confidentiality and adhere to data protection and information governance policies.
  • Collaborate with clinical and administrative teams to ensure data quality and consistency.
  • Work collaboratively with clinical, administrative, and management colleagues to support a cohesive and efficient team environment, contributing to shared goals and continuous improvement.
  • Train and support staff in the effective use of Anima, ensuring confidence and consistency in its application across the practice.

Carlton Group Practice is a well-established and progressive primary care provider located in the heart of Burton upon Trent. As the largest practice in East Staffordshire, we proudly serve a diverse patient population of over 17,500 across two sites: Carlton Street Surgery and King Street Surgery.

Led by a team of six dedicated GP Partners, supported by a Business Manager and an Operations Manager, our leadership shares a unified vision to advance the practice into the next phase of primary care development while maintaining exceptional standards of patient care.

Our clinical team includes five Salaried GPs, a skilled Clinical Pharmacy Team, Paramedic and an experienced Nursing Team. These are complemented by a strong Management Team and a committed group of administrative professionals who ensure the smooth running of our services.administrative staff.

The post-holder will handle sensitive and confidential information relating to patients, carers, staff, and the business of the practice. All such information must be treated with the utmost confidentiality and only shared with authorised individuals in accordance with practice policies and data protection regulations.

Person SpecificationQualifications

  • GCSE grade A to C in English and Maths

Experience

  • Experience of working in primary care
  • Knowledge of Read Codes and/or SNOMED CT
  • Familiarity with clinical systems (e.g. SystmOne)
  • Understanding of medical terminology and anatomy
  • Knowledge and understanding of safeguarding procedures
  • Attention to detail and accuracy
  • Experience handling sensitive information
  • Ability to work independently and as part of a team
  • Full UK driving licence and access to a vehicle (for working across two practice sites)
  • Training in clinical coding or medical terminology
  • Experience with QOF and NHS data reporting
  • Strong organisational and communication skills
