Reablement Enabler

Are you passionate about helping people regain their independence and live full, healthy lives in their own homes? We are looking for a dedicated Reablement Enabler to join our cients' Adult Social Care team.

This is a fulfilling role where you will provide intensive, short-term support to adults with diverse needs. Your work will focus on re-enabling people, helping them build on their strengths and achieve personal goals. This isn't just about providing care; it's about empowering individuals to do things for themselves again.

What you'll do:

Deliver personalised care and support based on reablement plans.

Work collaboratively with a multi-disciplinary team, including health and social care professionals.

Help people with personal care, domestic tasks, and therapeutic exercises designed by therapists.

Document and record progress towards reablement goals.

Adhere to a strengths-based, person-centred approach.

Be willing to work flexibly on a 7-day rota, including morning and evening shifts.

What you'll bring:

A Level 2 NVQ or QCF Level 3 in a relevant field.

Experience in domiciliary personal care within an Adult Social Care setting.

An understanding of the Care Act 2014 and safeguarding principles.

The ability to work independently and as part of a team.

A genuine commitment to promoting independence and empowering individuals.

Willingness to travel across our geographical area.
